Investigation of the relationship between gastric myoelectric activitymeasurements and gastric emptying rate in healthy and diabeticgastroparetic individuals

Aim: To investigate the relationship between gastric myoelectric activity and gastric emptying rate in healthy patients and patients diagnosed with diabetic gastroparesis (D-GP) and to evaluate the clinical use of EGG. Material and Method: Our study was conducted on a total of 40 male individuals, 20 of whom were healthy and 20 of whom were diagnosed with symptomatic D-GP. Gastric myoelectric activity was recorded by cutaneous electrogastrography (EGG). After 10 ml of fasting venous blood was collected from each participant, 30 min. preprandial and then 60 min after eating the standard test meal (450 kcal). Postprandial EGG recordings were taken. The next day, the same test meal was given to each individual who came hungry at midnight, and a gastric emptying rate (GER) test based on antral area measurement was performed using ultrasonography at the 15th and 90th minutes. Results: Fasting blood sugar and HbA1c values of individuals with D-GP were statistically significantly higher than the control group (p<0.001, p<0.001, respectively). In patients diagnosed with D-GP in the pre- and postprandial periods, the percentage of DF and Normogastri was lower compared to the control group (preprandial p<0.001, p=0.006, respectively; postprandial p= 0.034, p<0.001, respectively), and the percentage of bradigastria was higher (p<). 0.001). PPC value showed a non-significant decrease in diabetics compared to controls (p>0.05). Compared to the control group, significant antral dilatation and an accompanying significant decrease in gastric emptying rate were observed in gastroparetic individuals at the 90th minute postprandial (P<0.001). Ghrelin, motilin, GIP and GLP-1 levels measured in diabetics were lower than the control group. No correlation was detected between gastric emptying rate and FBG, HbA1c, most EGG parameters and hormone levels in both groups. However, a positive correlation was found between GER and PPC in the D-GP group. Conclusion: According to EGG records, gastric emptying was delayed in the diabetic gastroparetic group, which showed a decrease in both rhythmicity and postprandial dominant power change. Abnormalities in gastric myoelectric activity and antral dilatation may cause delayed gastric emptying. An EGG recording supported by antral area measurement may benefit the clinic in distinguishing diabetic gastroparesis patients.
