Abstract (EN)
Rotator cuff tears are a serious disease that causes both pain and loss of workload, which is increasingly common among young athletes and older populations. This disease is frequently treated surgically and the incidence and recurrence rates are very high. Autologous serum concentration has been shown to enhance tissue repair with cytokines and growth factors and has been shown to be effective in muscle and tendon injuries. Hyaluronic acid is a major component of joint synovial fluid and is found in tendon and ligament structures. Hyaluronic acid has an anti-inflammatory effect by regulating prostaglandin E2, C4S and interleukin 1 levels in the synovial fluid. It also reduces leukocyte migration, phagocytosis and lymphocyte proliferation, thereby reducing pain. It is also known to contribute to tendon healing by preventing adhesion. The aim of this study is to compare the effect of autologous serum concentration (ACS / Orthokine) and hyaluronic acid (Hyalone) on the healing process after rotator cuff repair in an experimental rotator cuff repair model. Winstar albino rats to be used in the study were obtained from the BEU Laboratory Animal Unit. The study was designed, Group A not having any surgery or medical therapy group, Group B will constitute the control group with supraspinatus tenotomy and repair. Group (C) is the ACS group with supraspinatus tenotomy and repair + 170 µm Orthokine injected to repair area. Group (D) is the hyaluronic acid group with supraspinatus tenotomy and repair + 0,1 ml/g Hyalone injected to repair area. After 4 weeks of follow up, subjects will be evaluated functionally using a motorized treadmill for their running performance (0-15 mn., 15-30 mn., 30-45 mn., 45-60 mn.). Running performances cores will be evaluated statistically. After functional evaluation, subjects will be sacrificed for histological evaluation. As a result of our study, Orthokine and Hyalone; neovascularization in tissue healing; fibroblast proliferation and fibrosis parameters, but they did not make any statistical difference. However, the Hyalone significantly reduced the fire, and as a result, the running performance increased significantly.
