Özet (EN)

In this study, it was aimed to determine the clinical findings of the patients who admitted to the emergency department due to synthetic cannabinoid (SC) use, to investigate the clinical, radiological and blood parameters that can guide to differential diagnosis, and to identify the patients who may develop possible brain damage. In our study, the files of the patients who admitted to Şişli Hamidiye Etfal Training and Research Hospital Emergency Medicine Clinic between 01.01.2016 and 01.01.2021 after SC use were examined. 538 patients using SC were included in our study. The patients included in our study were divided into three groups as follows; Group 1: all patients using SC, Group 2: patients among SC users underwent brain imaging with the suspicion of brain damage, Group 3: control group patients who admitted to the ED with minor head trauma or headache and underwent brain imaging. In our study, a significant correlation was found between low Glasgow Coma Scale (GCS) score and group 2 in terms of consciousness. When the ECG was examined, a significant difference was found between groups 1 and 2 in terms of tachycardia and increased QTc length. When examined in terms of hospitalization in the ward and intensive care unit, it was seen that the hospital and intensive care hospitalizations of the patients in group 2 were ⁓2 and ⁓3 times higher, respectively, compared to group 1. When the patients were examined in terms of PSS, it was seen that PSS score was in severe and fatal scale in approximately half of the patients in group 2. In addition, there were nine patients with brain damage (ischemic/hemorrhagic cerebrovascular event (CVE)) in group 2. The potential of SCs to impair vital functions and cause life-threatening situations has become an important problem for clinicians and toxicologists. Our research supports a causal link between SCs and SVE. The use of SCs, especially in young patients with brain damage or suspicion, should be questioned in emergency departments, and toxicological screening should be performed, if necessary. Future studies will be needed to clarify the role of SCs in brain damage. Key words: Brain imaging, Bonsai, Cerebrovascular event, Synthetic cannabinoid.
