Effect of 20cc Bupivacaine applied subcutaneously to the incision section on postoperative pain in cesarean section deliveries

OBJECTIVE: To investigate the efficacy of bupivacaine applied subcutaneously to the incision site on postoperative pain levels in patients who had cesarean section under general anesthesia. MATERIAL AND METHODS: The files and information of patients who had cesarean section operation under general anesthesia at T.C. Demiroğlu Bilim University Faculty of Medicine and at Private Ataşehir Avicenna Hospital, Department of Obstetrics and Gynecology between 01.01.2020 and 01.06.2022 were retrospectively reviewed. Data of 60 patients included in the study divided into two groups. Group A (n=30) consists of pregnant women who had 20 cc bupivacaine was applied subcutaneously to the incision line at the end of the operation and then post-operative pain scores were made with VAS. Group B (n=30) consists of pregnant women who had no application made to the incision line at the end of the operation and pain scoring was done with VAS after the operation. Postoperative 2nd hour, 4th hour, 6th hour and 12th hour pain levels were retrospectively analyzed. RESULTS: Pain levels at 4th, 6th and 12th hours in group A were found to be statistically significantly lower than those in group B (p<0.05). In addition, the 2nd hour pain level in group A was significantly higher than the 6th and 12th hours. Moreover, the 4th hour pain level was significantly higher than the 6th hour pain level. In the group B, the 2nd hour pain level was significantly higher than the 6th hour pain level, and the 4th hour pain level was significantly higher than the 6th and 12th hour pain level. CONCLUSION: In our retrospective study, we concluded that 20 cc Bupivacaine infiltration applied subcutaneously to the incision area at the end of the surgery in cesarean deliveries performed under general anesthesia is effective in post-operative pain management by reducing pain scores.
