Comparison of Bupivacaine and Bupivacaine+Fentanil under superficial cervical block for thyroidectomy and parathyroidectomy

We compared the clinical efficacy and postoperative pain of the bupivacaine and bupivacaine+fentanil in superficial cervical plexus block on elective thyroid and parathyroid surgery.ASA I-II, 18?70 ages 50 patients undergoing elective thyroid surgery divided into two groups randomly. Patients were monitorized in operative room. Grup B (n=25) 125 mg % 0,5 bupivacaine 25 ml+ 25 ml salin, Grup BF (n=25) 125 mg %0,5 bupivacaine 25 ml+ fentanil 2mcg/cc (2ml) +23 ml salin was given for superficial cervical plexus block. Sensorial block onset time was controlled by pinprick test. After the operation was started blood pressure, pulse, peripheral oxygen saturation, respiration rate, sedation score was recorded. This values was recorded until postoperative tree hours. After the operation was finished time to require analgesic was total analgesic time which was recorded.There was no significant istatistical difference between the two groups for the patients satisfaction, peripheral oxygen saturation, sedation score hemodynamy and side effects. Respiratory rate in bupivacaine+fentanil group intraoperative 5,10,15,30,45. minutes was significantly low, but clinically insignificant. After Operation started, hemodynamic parameters in both groups decreased after 5 minutes and increased after 30 minutes and have been starting values. Sensorial block onset time was short in bupivacaine+fentanil group (Grup B 98 sn ± 32 sn Grup BF 81 sn ± 24 sn ). Sensorial block finish time was long in bupivacaine+fentanil group (Grup B 306 minutes ± 108 minutes, Grup BF 385 minutes ± 125 minutes ). There was no difference between the two groups in duration of analgesic requirement. We didn?t found important side affect in both groups.As a result, we conclude that thyroidectomy and parathyroidectomy under bilateral superficial cervical plexus, 125 mg 0.5% bupivacaine 25ml + 25 ml salin 0.5% bupivacaine + fentanyl 2mcg/cc (2ml) + 23 ml salin than the short duration of onset, longer duration of the block and patient satisfaction were similar.
