Evaluation of LEFT ventricle contractile functions and investigation of their relationship with disease activation in patients with ulcerative colit and crohn patients

Aim: Inflammatory bowel disease (IBD) is a lifelong chronic immune-mediated disease characterized by periods of remission and relapse. Cardiac involvement of IBD is rarely seen. The aim of this study; It is to determine the early prediction of cardiac risks associated with inflammation in IBD patients. Materials and Methods: Patients with active disease, who were diagnosed with Ulcerative Colitis and Crohn's Disease, endoscopically, clinically and histopathologically, who applied to Gaziantep University Faculty of Medicine Gastroenterology outpatient clinic between August 2021 and February 2022, were included in this study. Longitudinal Strain Echocardiography was performed before treatment in patients with active disease. Control Longitudinal Strain Echocardiography was repeated in patients who achieved remission according to clinical activation indices after treatment, and GLSAvg data in both measurements were statistically compared. Results: There was no significant correlation between active and remission longitudinal strain measurements in the comparison group that included all patients (p=0.196). In the evaluation made in the ulcerative colitis group, no significant correlation was found between the active and remission longitudinal strain measurements (p=0.906). In the evaluation made in the Crohn's Disease group, there was no significant difference between the measurements (p=0.067). In the analysis made between clinical activation scores and strain measurements, no significant correlation was found according to Mayo score, CDAI and Harvey-Bradshaw indexes. In addition, while there was no significant difference between men and women in active period strain measurements (p=0.077), remission measurements in women were found to be significantly lower than in men (P=0.006). Conclusion: Although cardiac functions of IBD patients are affected, it has been suggested that there is no relationship between disease activity status and cardiac contractile functions. According to the results of the study, the significant decrease in strain values measured in remission in women compared to men suggested that there is a tendency to improve myocardial tissue deformation in women with IBD who achieved remission. According to the findings, it was determined that cardiac risks are increased in IBD patients who smoke.
