Comparison of transobturator tape (TOT) and kelly plication operations due to urinary incontinence

Urinary incontinence has a negative impact on women's social and hygienic quality of life. Although there are many methods for the surgical treatment of urinary incontinence, a gold standard method has not been determined yet. The aim of our study is to investigate the factors that may affect urinary incontinence and to compare the short-term results of TOT and Kelly plication operations, which are surgical methods used in the treatment of urinary incontinence. The data of 253 patients who underwent transobturator tape (TOT) and Kelly Plication operations for urinary incontinence in Gaziantep University Faculty of Medicine, Department of Obstetrics and Gynecology between January 2016 and December 2021 were analyzed retrospectively, considering their 1-year evaluations after the operation. The preoperative and postoperative information of the patients were analyzed using the Gaziantep University Faculty of Medicine information management system and archives. Patients' age, type of operation, complete urine analysis, urea, creatinine results, chronic disease status, menopause status, gravida and parities, body mass indexes, urinary incontinence type, positive stress test, vaginal prolapse degrees, post-operative short-term results were analyzed. The patients were divided into two groups as TOT and Kelly plication operations. Group 1 was determined as those who underwent TOT operation and Group 2 as those who underwent Kelly plication operation. Student's t-test was used for numerically normally distributed variables, and Mann-Whitney U test was used for non-normally distributed numerical variables. Data were evaluated using the chi-square test for categorical variables. All statistical analyses were explained via R statistical software, version 3.5.1 (R Statistical Software, Institute for Statistics and Mathematics, Vienna, Austria). In our study, patients who underwent TOT and Kelly operations were compared. A statistically significant difference was observed between group 1 and group 2 when the 3-month findings were examined in the 1-year period after the surgery. The success rate after TOT operation was higher than the group with Kelly plication. Considering the number of parity and gravida, there was a significant difference between the two groups, but there was no significant difference in age, BMI, menopausal status, urinary incontinence type, laboratory results, and chronic disease status. Considering the general population, it was observed that the incidence of urinary incontinence increased as age, gravida, parity and BMI increased, and postmenopausal status and urinary tract infection were important risk factors for urinary incontinence. TOT and Kelly plication operations are minimally invasive surgical techniques used in the treatment of SUI, and the short-term success of TOT was found to be higher in our study. It may be more beneficial to have prospective studies with a larger number of patients and variables to determine more successful surgical methods for the cause of SUI.
