Retrospective evaluation of the clinical and pathological features of patients with uterin sarcoma

Abstract (EN)

Introduction and aim: Uterine sarcomas (US) account for approximately 1% of all female genital tract malignancies and 3–7% of all uterine cancers. Because of their rarity and histopathological diversity, there is no consensus on poor prognostic factors and treatment. In this study, we evaluated the clinical and pathological features of patients with US who were followed up in our center. Materials and method: The medical records of 65 patients diagnosed with US between 2010-2019 years were investigated retrospectively. Patients' age, gender, date of diagnosis, operation date, pathological subtype, presence of metastasis, adjuvant treatment regimens, presence of relapse, treatment received after recurrence, and death status were recorded. Disease-free and overall survival analyzes were determined by the Kaplan-Meier method. In statistical analysis, p <0.05 was considered statistically significant. Results: The mean age of the patients was 58 years. The most common subtype was leimyosarcoma (%44.6). The most common presentation was vaginal bleeding. At diagnosis, 50 patients (%76.9) were at the local stage and 15 patients (%23.1) were at the metastatic stage. Median overall survival time in patients was found to be 76 months. The 5-year overall survival probability was determined as 52.7%. The prognosis was better in patients without comorbidities and in patients under 65 years of age. The prognosis in carcinosarcoma was worse than other subtypes. There was no statistically significant difference between the overall survival times according to disease stage, tumor grade, tumor necrosis, tumor diameter and lymph node involvement (p> 0.05). Conclusion: Apart from benign causes, the possibility of US should be kept in mind in patients presenting with a vaginal bleeding and pelvic pain and tissue sampling should be performed. The development of metastasis negatively affected the survival during the disease. Patients should be careful about regular follow-up after diagnosis and early treatments.
