Endovascular treatment in orificial occlusive lesions of vertebral artery

Purpose: Vertebral artery stenosis is found in 20% of patients who had posterior fossa ischemia. Endovascular treatment process has become more preferable to use in the treatment of vertebral artery orificial occlusiv lesions due to the developments in the last 20 years. In this study, we aimed to present the clinical results and show the success of the endovascular treatment in occlusiv lesions localised in VA orifice.Material and method: In this study, endovascular treatment was performed on 29 lesions in 28 patients, who were taken to the hospital due to the clinical symptoms and diagnosed with vertebral artery orificial occlusive lesions by radiological imaging modalities. The 35.7% of the population female (10) and 64.3% was male (18). The average age of the population is 58,3, raging from 22 to 80. After treatment, patients follow up was scheduled on 1. day, 1, 3, 6, 12. months and per year afterwards by Doppler US and clinical finding and also angiographic findings 5 patients.Results: Endovascular treatment was applied to 19 patients with left vertebral, 8 patients with right vertebral, and 1 patient with left and right vertebral lesions. Technical success rate was 100%. During the procedure and at early term no complication was seen. 1 total occlusion, 3 preocclusiv stenosis and 1 75% stenosis were seen during follow up. 2 of the patients with preocclusiv stenosis was performed with endovascular treatment again. For early (0-3 months) term, primary and secondary patancy rate was 100%. For mid (4-6 months) term, primary and secondary patency were 96.4%. We found primary patency rate 86.9% and, secondary patency rate 91.3% for long term (7-12 months)Conclusion: Endovacular treatment, combined with optimal medical therapy is an effective treatment method in orificial occlusiv lesions of vertebral artery.Keywords: Vertebral Artery Orifice, Endovascular treatment, Stent, Stenosis, Occlusion.
