Abstract (EN)

Various parameters have been shown to predict prognosis and predict overall survival in various types of cancer. Extensive stage small cell lung cancer is a malignancy with an extremely low survival rate, and there are few studies on parameters that are indicators of survival and prognosis. mGPS and GPS are based on serum albumin level and CRP. PNI is a combination of serum albumin level and absolute lymphocyte count. CAR is calculated by the ratio of CRP and serum albumin and has been found to be a prognostic factor in various cancer types in many studies. In this study, 144 patients who were pathologically diagnosed with SCLC and had extensive stage between 01 January 2011 and 1 June 2022 were included. Scoring was calculated with the height, weight, biochemical parameters and complete blood count of the patients at the time of diagnosis. The cut-off was determined by performing roc curve analysis for PNI and CAR. In the survival analysis, the overall survival of patients with mGPS 0-1, GPS 0-1 and higher PNI was found to be significantly longer than mGPS 2, GPS 2 and lower PNI. CAR value was not significant in terms of survival. High PNI (p=0,018), mGPS 0-1 (p=0.007) and GPS 0-1 (p=0.007) were defined to be good prognostic factors in univariate analyses. In multivariate models, high PNI (p=0,015), mGPS 0-1 (p=0,026) and GPS 0-1 (p=0,026) were determined as independent prognostic factors. Our study was a single-center retrospective study; however, data suggest that pre-treatment mGPS, GPS and PNI are independent predictors of overall survival in patients with extensive SCLC. Pre-treatment evaluation of mGPS, GPS, and PNI may be useful for identifying patients with a poor prognosis.
