Essential hypertension pathogenesis hasincreased intravascular fluid volume, sympathetic hyperactivity, increased activity in renin angiotensin aldosterone system (RAAS) the due to vasoconstriction, reducted vasodilator effect of prostaglandins and nitric oxide (NO) increased vasoconstrictor effect of endothelin and at the result of increased the peripheral vascular resistance. An increase in cardiac output causes thatsodium retention, increase of intravascular volume and anemia. By definition endothelial dysfunction in endothelial cells, nitric oxide (NO) bioavailability and oxidative stress caused by a defect in the endothelial cells is functional and reversible modification. In our study,we aimedto research at the endothelial dysfunction among microalbuminuria, plasma levels of NO and endothelin 1 in hypertension. The relationship between the dippers and non-dippers in patients with hypertension plasma NO, endothelin-1 levels and the relationship 24-hour urinary electrolyte levels with microalbuminuria. In our study of newly diagnosed hypertensive any medicated before 54 patients were 27 females and 27 males. Patients without and with endothelial dysfunction among plasma NO, endotelin-1 levels, ambulatory blood pressure measurements, 24-hour urinary Na (UNa), potassium (UK), chloride (UCl), calcium (UCa), magnesium Mg, creatinine, microalbuminüriaand creatinine clearance were measurement.Subgroups analyzes were compared withobtained the data. We foundthat a significant change in systolic blood pressure and diastolic blood pressurewith high levels in patients with endothelial dysfunction, which is an indicator of microalbuminuria values significantly higher in patients with hypertensive (105.6±161.9 versus 112.6±172.2mg/day).We have found that not a significant differences between plasma NO (71.24±51.48 µmol/L) and endothelin-1 levels (2.07±1.5pg/ml), dippers and non-dippers revealed 24-hour urinary electrolytes excretion in patients with prehypertension (respectively, p = 0.805 for Na, p = 0.421for K, p = 0.872 for Cl, p = 0.144 for Ca, p = 0.813 for Mg and p = 0.675 for CCr). As a result of reduction in NO levels and ongoing microalbuminuria that the markers of endothelial dysfunction and may be associated with essential hypertension.
