Özet (EN)

ABSTRACTPurpose: This study is performed to determine the clinical and laboratory features of the patients with rheumatologic diseases admitted to the Department of Medical Intensive Care of Cukurova University Medical Faculty and to investigate the factors associated with mortality.Material and methods: Patients admitted to the Department of Medical Intensive Care, with rheumatologic disease, between 2003 and 2012 were included. The demografic, clinical and laboratuary datas of patients on admission were recorded retrospectively. Frequency of datas and prognostic factors associated with mortality were evaluated.Findings: One hundred patients, 69 female, 31 male, with median age of 38(14-83) were included in this study. Fourty two (42%) patients had Sistemik lupus eritromatozis (SLE), 26 (%26) patient had Romatoit artrit (RA), 9 (%9) patients had Wegener?s granulomatosis (WG), 6 (%6) patient had Behcet disease(BD), 5 (%5) had Scleroderma (SCL). Fifty six (%55) of the patients admitted to ICU because of respiratory failure and shock, 27 (%27) admitted because of neurologic problems and 18 (%18) admitted with other reasons. Median Acute Physiology and Chronic Health Evaluation (APACHE II) score was 21(5-45)63 (63%) patients died . in ICU. Admission diagnosis (p=0.002), age (p=0.040), infection at admission (p=0.005), pneumonia (p=0.017), sepsis (p=0.01), shock (p=0.01), platelet count (p=0.032), C-reaktif protein (CRP) (p=0.012), procalcitonin (p=0.008) and APACHE II (p=0.001) score were found to be related with mortality in univariant analysis. In multivariant analysis only Acute Physiology and Chronic Health Evaluation (APACHE II) were significantly related with mortality(p=0,009).Mortality rate of 42 SLE patients was 63%. Age(p= 0.002), sepsis (p=0.028), APACHE II (P=0.006) and, procalcitonin (p=0.046) were related with mortality. Mortality rate of RA patients was 73%. Only CRP (p=0.028) was significantly related with mortality in RA patients.Conculusion: Patients with Rheumatologic disease have high mortality rate in ICU. APACHE II score is most important risk factor for mortality for rheumatologic patient in ICU
