Abstract (EN)

Prostate cancer is a major cause of disease and mortality among men, and each year 1.6 million men are diagnosed with and 366,000 men die of prostate cancer (1) High-risk prostate cancer is a potentially fatal disease and occurs in 14-24% of newly diagnosed cases. In men with localized prostate cancer and a life expectancy more than 10 years the gold standard treatment option is Radical Prostatectomy (RP). Patients with (PSA) ≥20 ng / ml, (GS) 8-10 or clinical stage ≥T2c are considered high-risk. Robotic Assisted Laparoscopic Radical Prostatectomy (RALRP) is performed more and more often by surgeons while the value of minimal invasive surgery increases in this area. The study and follow-up of postoperative functional and oncologic outcomes is very important for the determination of future planned treatments. In our study, functional and oncologic outcomes of patients with robotic prostatectomy who had been diagnosed with high and advanced prostate cancer were studied. In our study, the functional and oncological results for 1100 patients who were diagnosed with high-risk prostate cancer and had RLRP between November 2010 and October 2017 by two surgeons gained a certain experience were investigated and compared with the literature. In this study biochemical recurrence-survival rate (BCR) PSA value were defined as ≥ 0.2 ng/ml. The mean follow-up period of 18 months (12-24 months) was 62% with no biochemical recurrence and 22% had PSA recurrence (BCR). In addition, the PSM rate was 38%. Postoperative functional outcomes (potency and incontinence status) of the patients were also evaluated , According to our results, radical prostatectomy should be recommended in patients with high grade PC and life expectancy more than 10 years. Although RLRP performed by experienced surgeons maybe does not affect oncologic results, but we believe that we can achieve more satisfactory results in functional results, and to verify this, it is necessary to scan and handle larger patient series data.
