Dermatoscopic and histopathologic correlation in macular hyperpigmented lesions which are placed on the face

Actinic keratoses, seborrheic keratoses, solar lentigo, lentigo maligna are hyperpigmented lesions which occur with the effect of ultraviolet in sun exposed areas and may be confused with each other. It is difficult to distinguish these pigmented lesions clinically, occasionally impossible. Dermatoscopy is an important tool to distinguish the pigmented lesions which are placed on the face. In this study we aimed to determine the reliability between dermatoscopic and histopathologic findings of hyperpigmented flat lesions such as actinic keratoses, seborrheic keratoses, solar lentigo, lentigo maligna which were placed on the face.Patients who admitted to Department of Dermatology of Cukurova University Medical Faculty having hyperpigmented flat lesions on the face were enrolled to study. In evaluation of the cases we recorded the dermatoscopic criteria form which was prepared for the seborrheic keratoses, actinic keratoses, solar lentigo and lentigo maligna. Biopsy was taken for histopathologic evaluation.In the study 51 patients 26 males (% 51) and 25 females (% 49) with 53 skin lesions were evaluated. The ages of the patients ranged from 40 to 83 years (median 64,47 ± 11,24). We did not statistically evaluate the 3 lesions which had histopathologic diagnosis of actinic keratoses + solar lentigo. Rest of the 50 lesions histopathologic diagnoses were actinic keratoses in 32 lesions, seborrheic keratoses in 9, solar lentigo in 9, respectively. The statistical analysis was done with Kappa test. Kappa test result was found 0,645 (p<0,001). In the diagnoses of the hyperpigmented flat lesions which were placed on the face, dermatoscopic and histopathologic diagnoses were found statistically moderately concordant according to the Kappa values.Keywords: Actinic keratoses, dermatoscopy, seborrheic keratoses, solar lentigo.
