Analysis of patients who admitted to the emergency service with 112 first aid service in 6 months period in 2016-2017

Abstract (EN)
INTRODUCTION AND AIM: Turkey is a country where emergency diseases, natural disasters, accidents and injuries are frequently experienced. For this reason, the development and organization of emergency health services is of great importance. In this thesis study, the patients brought with 112 Emergency Ambulance Services were examined; The evaluation of the demographic characteristics of patient applications were aimed. And also it was aimed to evaluate the effects of 112 ambulances on Emergency Service intensity while planning the future of the emergency services. MATERIALS AND METHODS: This study is a cross-sectional study and the subjects of this study are the ones brought to Adnan Menderes University Hospital Emergency Department by 112 Emergency Service ambulance between 01 September 2016-31 December 2016. Patients' information was obtained from 112 emergency service ambulance form and hospital registry system. The sample of the research was composed of 1066 people whose data were available and who were volunteered. The data of the study were collected by using the prepared data collection form by us and the 112 emergency ambulance data form. The data collection form consists of twelve sections. With the data collection form, the patient's wristband barcode, vital signs of the patients (blood pressure arterial, pulse, fever, oxygen saturation), arrival time with the 112 ambulance, arrival day with the 112 ambulance, Glasgow Coma Score calculated in 112 ambulance, interventions made during the 112 ambulance transportation(IV catheterization, Airway intubation), 112 emergency ambulance preliminary diagnosis, hospital definite diagnosis, clinical course of patients (outcome after emergency department visit, hospital general outcome), service or intensive care hospital stay were recorded. The data were recorded by the researcher in the data collection form and evaluated in the statistical program named SPSS afterwards. Descriptive statistical analyzes are indicated using median (minimum-maximum), mean ± standard deviation, frequency, percentage. Kolmogorov Smirnov test, Chi-square test, Student T test and Spearman correlation analysis were used for analytical statistical analysis. RESULTS: 52.3% (n: 558) of the patients included in the study were male, 47.7% (n: 508) were females. The median age is 52.0 (1.0 -112.0), the average age is 49.75 ± 24.99. Patients were mostly brought on saturdays (15.4%) and least on mondays (12.8%). Patients were most frequently brought between 09.01-16.00 hours (44.3%) (n = 472) and least between 00.00-09.00 hours (18.9%) (n = 201). According to the preliminary diagnosis of 112 emergency ambulances, the most frequent patient group which brought by 112 ambulance was trauma patients ( n= 335, % 31,4), second frequent group was cardiovascular system emergencies ( n= 169, % 15,9 ), third frequent group was gastrointestinal system emergencies ( n= 131, % 12,3 ), and fourth frequent group was neurological emergencies ( n= 114, % 10,7). 67.9% (n = 724) of the patients who were brought to the emergency department by 112 emergency ambulance were discharged after the first evaluation, 0,7% (n = 7) of the patients were referred to other hospitals, 0,5% (n = 5) of the patients were resulted in exitus. 209 patients were hospitalized in various services after the emergency department evaluation. The most frequent 3 diagnoses of the patients who hospitalized in services were trauma emergencies (22.5%), neurological emergencies (13.9%), and pulmonary emergencies (12.0%). 11,10% (n = 118) patients were hospitalized into the intensive care units after being evaluated in our emergency department. The most frequent 3 diagnoses of the patients who hospitalized in intensive care units were cardiovascular system emergencies (%30,6), pulmonary emergencies (24%) and gastrointestinal system emergencies (12,4%). CONCLUSION: The majority of the patients who brought by the 112 emergency ambulance were discharged. This suggests that 112 emergency ambulances are unnecessarily used. This problem can be solved by making the society aware of this issue. If it is considered that the most common diagnosis is trauma in patients who brought by 112 emergency ambulances, planning to increase the knowledge and experience of 112 emergency ambulance crew on prehospital trauma can be a good idea. While staffing the emergency services, appropriate planning can be made by considering the hours when 112 emergency ambulances are most frequently applied. The bed capacities of the departments where the most frequent service and intensive care hospitalizations are made should be observed.
