Abstract (EN)

Objective: To determine the value of CRSI-65, CURB-65 and CURSI scoring systems in predicting 28-day mortality and intensive care unit admission in COVID-19 patients Method: Patients with positive COVID-19 PCR test and covid pneumonia finding on thorax CT who applied to Ankara City Hospital Adult Emergency Medicine Clinic were presented with information about their application, demographic information, symptoms, vital signs, clinical features, laboratory results and imaging findings. The CRSI -65 score was evaluated. It is a Prospective Observational Cross Section Study. Results: 540 (100%) patients were included in our study. Of these patients, 314 (58.1%) were male and 226 (41.9%) were female, with mean age (mean±sd) 65±15 and median (median[min-max]) 66 [22-99] years found. The number of patients who died within 28 days was 88 (16.3%) and the number of patients hospitalized in the intensive care unit was 157 (29.1%). All risk scores were significant in predicting mortality and intensive care unit admission. ROC analysis for risk estimation was 0.821 (95% [CI] 0.771-0.871) for CRSI-65, 0.849 (95% [CI] 0.802-0.896) for CURB-65, and 0.842 (95% [CI] 0.795-0.889) for CURSI. For CRSI-65, the low-risk group included 0-1 points and the high-risk group included CRSI ≥ 2 points. Conclusion: In our study, although the CRSI-65 score was significant in predicting 28-day mortality and hospitalization in the intensive care unit between the low and high-risk groups, no superiority was found compared to CURB-65 and CURSI. However, the fact that it makes similar predictions with physical examination and vital signs without using any laboratory parameters suggests that it is advantageous.
