Abstract (EN)
Background and Aims: The aim of this study is to evaluate the relationship between left atrial remodelling parameters and thiol/disulfide homeostasis which is a new oxidative biomarker, and their effects on atrial fibrillation. Recent studies showed that there is a relationship between increased risk of atrial fibrillation and left atrial remodelling with oxidative stress. Material and Method: One hundred and twenty three patients who applied to our clinic between Marc 2018 and August 2018 and accepted to be voluntered, who were diagnosed with atrial fibrillation in 12 derivation ECG or ECG holter, were included in this research. Fifty eight healthy volunteres were selected as control group. A total of 181 cases were evaluated. ECG was performed, routine blood samples as well as thiol paramaters were assessed in all cases. The left atrial diameters and left atrial volumes were evaluated in echocardiography. These parameters were used to calculate the left atrial sphericity index and left atrial volume index. Results: Sixty eight percent of the cases were diagnosed as AF and thirty-two percent were control cases. Mean age was 68±12 years in AF patients and 57±13 years in control group. 2D echo and echo parameters that assess the basal measurement of the heart and left atrium remodelling was found to be statistically significantly higher in AF group. There wasn't any significant difference between non-valvuler AF and valvular AF, and paroxysmal and chronic AF in terms of serum thiol levels. Native thiol level was found to be negatively correlated with longitudinal left atrium diameter. Total thiol level was found to be negatively correlated with LA dimention index. Linear regression analysis showed that serum native thiol level, body surface area and AF time were independent markers for left atrial volume index. Conclusion: In terms of serum thiol/disulfide levels, there wasn't any statistically significant difference between AF patients and control group. Echocardiographic measurements which assess left atrial remodelling was significantly different in atrial fibrillation patients compared to control group. Also in our study showed that one of the independent determinants for left atrium volume index is serum native thiol level. This indicates that there is correlation between increased voume index and descreased native thiol level and it shows the effect of oxidative stress on left atrial volume. Increased left atrial volume index is an indicative for mortality and in the future, levels of native thiol and total thiol can be usefull as a biomarker for mortality.
