Abstract (EN)

Aim: We aimed to investigate the effects of pheniramine, meperidine and dexmedetomidine on sedative, hemodynamic and early postoperative cognitive functions in patients who are undergoing cataract surgery with local anesthesia. Methods and materials: In our prospective observational study, 112 patients with ASA I-III between 30 and 80 years of age who had a cataract operation planned under local anesthesia and sedation in the operating room of ADÜTF, were included. Routine monitoring was performed during preoperative, intraoperative and postoperative periods. Sedation levels preoperative, intraoperative 20.minutes were evaluated with Ramsey Sedation Score(RSS) in postoperative recovery room. Preoperative period, postoperative 24.hours underwent a Mini Mental Test for cognitive functions. Demographics, hemodynamic data of patients were recorded in the preparation room and sedation was performed. The meperidine group was given 0.5-0.7 mg/kg, Pheniramine group 0.7-1 mg/kg, dexmedetomidine group 0.6 mcg/kg at the loading dose as a 10-minute infusion. Hemodynamic data(OAB, HR, SpO2), side effects were recorded every 10 minutes during intraoperative and postoperative periods. Findings: There were no differences between the groups in terms of demographics. During the intraoperative period, RSS was higher in patients in the dexmedetomidine group compared to patients in the pheniramine and meperidine group, resulting in deeper sedation (p<0.05). In addition, pulse and blood pressure were significantly reduced in the dexmedetomidine group compared to the other two groups. MMT results in preoperative and postoperative periods were similar in all three groups. It was observed that all three drugs had no significant effect on postoperative cognitive function in the doses used. Conclusion: We believe that dexmedetomidine provides better sedation compared to pheniramine and meperidine, has no adverse effect on cognitive functions in the early 64 postoperative period, but can be used for cataract surgery considering that it can decrease pulse and blood pressure.
