Comparison of tissue adhesive and suture repair in noncomplete simple hand lacerations: Randomise controlled study

Abstract (EN)
The main purpose of wound closure is to improve the wound with a functional and cosmetically acceptable scar avoiding infection. The aim of this study was to evaluate the use of tissue adhesives and sutures in uncomplicated simple hand lacerations; Treatment time, patient comfort, improvement status, which is more useful to compare. A prospective, observational, single-center, and cross-sectional study was conducted at Manisa Celal Bayar University Hospital in Hafsa Sultan Hospital between May 2015 and January 2017. Simple hand cuts of 4 cm in length were included in the study. A total of 100 patients who agreed to participate in the study and received their consent were randomly distributed to both groups. Medical record numbers, demographic information such as age, sex, laceration position, laceration length-shape-depth, time after injury, injury mechanism, injury type (sharp-blunt) and foreign body suspicion, used suture material, Processing time, the pain (0 no pain, 10 the most severe pain) felt by the patient with the scale (VAS) during the procedure, the development of infection detection on the 2 nd and 10 th day controls, and wound healing on the 30 th day were recorded. Infection was not detected on the 2nd and 10th day of the patients. On day 30 controls, improvement was assessed between the two groups. A statistically significant difference was found between the two groups at 30th day control (p = 0.001). The mean VAS pain scores during the procedure were 2,02 in the tissue adhesive group and 2,70 in the suture group (p <0.001). When the processing times are compared; The mean value of tissue adhesives used was 3,26 minutes whereas the mean value of sutures was 9,72 minutes (p <0.001). When the healing rate is compared after laceration formation mechanism and applied technique; there was a significant difference between the two techniques in smooth-edge lacerations (p=0.001). When the recovery rate is compared after laceration localization and applied technique; there was a significant difference between the two techniques at finger-cuts without joint level (p = 0.018). When compared with tissue adhesive use versus suturing; tissue adhesive has been found faster, less painful, better healing technique. In appropriate traumatic hand lacerations, tissue adhesive can be used as an alternative method of suturing at emergency department.
