Investigation the relation between insulin resistance and serum vaspin levels in chronic renal failure

Abstract (EN)
Insulin resistance and insulin secretion abnormalities in uremic patients are thought to be responsible for atherosclerotic complications and cardiovascular events. In patients with chronic renal failure(CRF) cardiovascular events are the major causes of the deaths. Vaspin is a new member of adipocytokines family. In this study we aimed to determine the vaspin levels and the relationship between vaspine and insulin resistance in stage 3-5 CRF patients.This study includes 33 stage 3-5 CRF patients and 22 healty control subjects. In these study groups in addition to routine blood parameters vaspine levels and waist circumference and body mass index(BMI) parameters were evaluated. After the centrifugation of the blood specimens, serums were stored at -20 celcius degree and afterwards vaspin levels were evaluated by ELIZA method. Insulin resistance was determined by Homeostasis model assesment insulin resistance (HOMA-IR) method.There were no statistically significant difference between groups in terms of average age, body mass index (BMI) ,waist circumference and serum glucose level. Insulin levels (p<0.05) and HOMA-IR indexes (P<0.001) were significantly higher in CRF patients in comparison with control group. Comparison in terms of lipid parameters; trigliserid levels were higher in CRF group than control group; but this increase was statistically unsignificant. Total cholesterol, LDL and HDL levels were all lower in CRF group than control group but only decrease of HDL levels was statistically significant (p<0.05). In CRF group vaspin levels due to insulin resistance was statistically significantly different than control group (p<0.001). No relationship was determined between vaspin levels and anthropometric and metabolic values. Serum vaspin levels were positively but statistically un significantly correlated with fasting plasma glucose levels and age. There was no correlation between serum vaspin levels and HOMA-IR index.In conclusion, insulin resistance and insulin secretion abnormalities in CRF patients are thought to be responsible cardiovascular complications. According to this study levels of vaspin that is an adipocytokine released by visceral adipose tissue increases as a response to insuline resistance and due to the decreased renal excretion. In order to support our findings studies with more patient quantity are necessary.Keywords: Chronic renal failure, vaspin, insulin resistance.
