Investigation of histopathological examination in patients operated due to uterine myoma in terms of uterine sarcon stump benign myoma and its variants

Aim: Uterine leiomyoma, with its high incidence, particularly among women of reproductive age, is the most common indication for hysterectomy. Studies have shown the possibility of malignant pathologies in the histopathological examination. In our research, we aimed to investigate the occurrence rates of uterine sarcoma, STUMP (smooth muscle tumor of uncertain malignant potential), and benign myoma variants in patients operated for uterine leiomyomas. Method: Patients who underwent hysterectomy or myomectomy due to uterine leiomyoma at our hospital between January 2013-January 2023 were included. Retrospectively, demographic and clinical data of patients, chronic dissease, previous surgical history, complaints, Pap-smear results, ultrasound measurements of myoma size, endometrial biopsy, blood tests, surgical approach, final pathology results. Results: A total of 504 patients, with a mean age of 35.8±5.8 years (min:17-max:54), were included in our study. Demographic, clinical, radiological, and post-surgical pathology reports of these patients were reviewed. The rate of detection of STUMP was determined as 7.7% (n=39). Among these patient and age, marital status, BMI (body mass index), gravidity, parity, abortions, comorbidities, history of previous surgeries, HPV (Human Papilloma Virus) status, myoma size, and endometrial biopsy results were individually examined. However, no statistically significant relationship was found between the occurrence of STUMP and any of these mentioned characteristics. Conclusion: Uterine myomas generally have benign pathology, and STUMP and uterine sarcomas can be detected at a small rate. For this reason, myomas should be evaluated individually for each patient. We recommend close follow-up of patients to prevent bad situations in the future.
