Abstract (EN)

One of the most common problems after cardiopulmonary bypass (CPB) is postoperative atrial fibrillation (PoAF). PoAF; In addition to the increased risk of stroke, cardiac arrest and congestive heart failure, it may cause negative consequences by prolonging long-term intensive care and hospital stay. Extracorporeal surface, circuit elements, perfusion management, surgical procedure, drugs, anesthesia, hypothermia and normothermia play an important role in the formation of PoAF after CPB. The aim of this study is to examine the PoAF status in patients who underwent open heart surgery with the normothermic and hypothermic CPB method and to investigate the effects of these methods on the formation of PoAF. Fifty patients were included in this retrospective study, and the files and electrocardiographic images of patients who underwent normothermic and hypothermic CPB were examined. The patients in our study were divided into 2 groups as 25 normothermic (35-37 ℃) and 25 hypothermic (28-32 ℃) CPB patients. Data were created using the information in the patient files and ECG images. Then, the relevant data in terms of PoAF were evaluated by statistical analysis. The data obtained in our study were not statistically significant (p>0.05). However, temperature, ACLAMP and total CPB duration were found to be higher in patients with PoAF. In addition, it was determined that PoAF was higher in men, the average age of patients with PoAF was lower, more POAF was seen in patients who had valve surgery, and patients with the least PoAF were those who had undergone septal defect surgery. In our study, the rate of PoAF observed after CPB performed with hypothermia was lower, while the rate of PoAF observed after CPB performed with normothermia was higher. However, these differences were not significant (p>.05). In conclusion, PoAF after CPB is an undesirable and important adverse event. There is uncertainty regarding optimal temperature management for PoAF in patients undergoing cardiovascular surgery with CPB. Adequate and robust randomized controlled trials should be encouraged to fill this knowledge gap.
