Abstract (EN)

Comparison of the effects of the CORM-3 molecule with Gingko biloba, NAcetylcysteine and Pentoxifylline in a rat lower extremity acute ischemiareperfusion model. Objective: Ischemia is the decrease or failure of blood flow to tissues due to various reasons. Acute limb ischemia is one of the most common peripheral vascular emergencies known, with high morbidity, mortality, and costs. Reperfusion is the restoration of blood flow to ischemic tissue. In acute ischemia-reperfusion injury that occurs in the extremities, skeletal muscles have special importance. In this study, the effects of the CORM-3 molecule in the lower limb acute ischemia-reperfusion injury model were compared with the effects of Ginkgo biloba extract, N-acetylcysteine and Pentoxyphilline drugs. Methods: In the study, 56 Sprague-Dawley rats were divided into 7 different groups with 8 rats in each group. Group 1 was planned as the control group, Group 2-3- 4-5-6 was planned as ischemia-reperfusion groups and initially, ischemia was performed for 4 hours with a tourniquet. After the Esmarch bandage, which was wrapped starting from the most distal of the extremity, namely from the toes, was terminated at trochanter major level, the locked cable tie was evenly locked in the lower right extremity the desired ischemia pattern was achieved. No injections were given to Groups 1 and 2; Group 3-4- 5-6 were intraperitoneally injected with CORM-3, Gingko biloba, N - acetylcysteine and Pentoxyphilline, respectively, without opening the tourniquet at the end of 4 hours. Intraperitoneal drug injection was applied to Group 7, approximately 1 hour before the ischemia-reperfusion injury in order to evaluate the prophylactic effect of the CORM-3 molecule and ischemia was performed with a tourniquet for 4 hours. The results were analyzed according to biochemical, videoangiographic and pathological criteries. Results: In biochemical studies, there was a significant difference between the mean values of CK and myoglobin in Group 2, i.e. I/R, 22.621, 00 and 305.19 U/L, and G7 - G2 (p<0.001) for both enzyme measurements. When looking at the average of LDH enzyme values, LDH value was lower than Group 2 in all groups in which treatment v agents were used but there was no statistically significant difference between them. In videoangiographic examinations, there was a significant difference in the right/left perfusion distributions in all groups except Group 1, which did not have ischemia. When the right / left limb proximal and middle region results were evaluated, there was a significant difference between G2 and G3, G6 and G7. When the right / left limb distal was evaluated, there was a significant difference between G2 and G3, G6 and G7 in the extremities without ischemia, while there was a significant difference between G2 and G3 and G7 in the extremities without ischemia. In pathological examinations, there was a significant difference between G7 and G2 in all criteria. Conclusion: There is an increase in perfusion in medium and small diameter vascular angiosomes of the extremities that are ischemic by ischemic conditioning of the extremities, and in large, medium and small vascular angiosomes of the extremities that are not ischemic. While CORM-3 molecule increases perfusion in large, medium and small diameter vessel angiosomes at both prophylactic and therapeutic doses, the pentoxifylline molecule at therapeutic doses increases perfusion in large and medium vascular angiosomes.
