Incidence of hypertension and associated factors in children with unilateral multicystic dysplastic kidney disease

Abstract (EN)

Purpose: In this study, it is aimed to determine the characteristic features and incidence of hypertension and also associated risk factors (renin, angiotensin, aldosteron, blood chemistry) in children with unilateral multicystic dysplastic kidney disease.Material and Methods: The study was performed in pediatric nephrology department of Çukurova University Medical Faculty between March 2008 and August 2010. In the first step of the study, data of eighty seven patients with unilateral multicystic displastic kidney disease who have been followed between the years of 1993 and 2010 were investigated retrospectively.In the second step, twenty two voluntary patients with unilateral multicystic dysplastic kidney disease and twenty two healty children with similar sex and age were included into the study. ABPM was performed to all of children and incidence of hypertension was investigated. Blood and urine samples were taken from two groups. Blood chemistry parameters, plasma renin activity, aldosteron, angiotensin and also urine protein and microalbumin were measured.The statistical analysis was made in Çukurova University, Faculty of Medicine, Department of Biostatistics.Results: Plasma renin activity, aldosteron and potassium levels and incidence of hypertension were statistically higher in patients with unilateral multicystic kidney disease compared to healthy group. Other parameters were not statistically different.Conclusion: Incidence of hypertension was found statistically higher in patients with unilateral multicystic dysplastic kidney disease in our study. Hypertension was associated with renin-angiotensin-aldosteron system. Further studies are needed to respond to the questions searching which is more effective in hypertension; sick kidney or healthy compensatory kidney and also incidence of hypertension can be reduced with nephrectomy. However, it is noticed that prevalent studies are not satisfactory to evaluate ambulatory blood pressure monitorization in children.
