Abstract (EN)

ABSTRACTObjective:Thyroid gland nodules are the most common disease of the thyroid gland . The most important concern for patients that have nodules is that if they have cancer or not. The prevalence of the detection of cancer in thyroid nodules is 5-15%. Today, fine-needle aspiration biopsy is the basic diagnostic method (FNAB). However, FNAB has low sensitivity in the evaluation of follicular lesions. We evaluated patients whose nodules were reported as follicular neoplasm, Hurthle cell neoplasm and have oncocytic changes in FNAB results. We compared the patients? ultrasonographic findings, age, gender, history of goiter, goiter type, cancer, smoking and a history of neck irradiation, scintigraphic features of nodules with postoperative histopathologic examination results.Materials and Methods:Our study included the patients that applied to University Hospital of Düzce Medical Faculty?s Endocrinology Clinics and Internal Medicine Clinics between the years 2009 and 2012. All the patients that were included had underwent FNAB and diagnosted as having folicular neoplasm, oncocytic changes and Hurthle cell neoplasm. These patients had underwent thyroidectomy operation. Malignant and suspiciously malignant cytology results were excluded. Patient files were retrospectively reviewed. 81 women and 22 men, a total of 103 patients, between the ages of 26-78 were eligible fort he study. The age, gender, history of62goiter, a history of cancer, neck radiation, smoking history, goiter type (solitary nodular, multinodular goiter), nodules? ultrasonographic features (echogenicity, long size, calcification), scintigraphic features of the nodules, FNAB results (for each nodule) and postoperative histopathological examination of thyroidectomy material results were recorded.Results and Conclusion:The mean age calculated for all patients was 48.6±11.1 ; 47.5±11.1 for women only; 52.9±10 for men only. Follicular neoplasm in 71 cases, Hurthle cell neoplasm in 10 cases, oncocytic changes in 22 cases were reported in FNAB cytologic evaluation. All patients were evaluated for the postoperative histopathological results. 15 cases were papillary carcinoma, 6 cases were follicular carcinoma and 82 cases? histopathological results were benign. The difference between the results of postoperative histopathology and the results of FNAB were not statistically significant. Ultrasonographic features of nodules and postoperative histopathology results were compared. There was a statistically significant relation between the histopatology results and echogenicity of nodules. As for age, gender, calcification, nodules? long size and short-and long-aspect ratio, scintigraphic features there was no significant difference. We concluded that for a thyroid nodule, being sonographically hypoechoic is an independent predictive factor for malignancy.
