May Tromboelastography be helpful for the evolution of coagulation disorders in patients with sepsis?

Fifty-five patients admitted in Intensive Care Unit of Anesthesiology and Surgery of Faculty of Medicine of Celal Bayar University, were included into this prospective study. Patients were divided into two groups, sepsis group (n=21) and control group (n=34). Group of patients with sepsis was created from patients diagnosed with SIRS-SEPSIS-MODS in intensive care unit. The patients? age, sex, admission in which day, which day of ventilatory suppert was used, the diagnosis, the last blood cultures, APACHE II score, whether the drug will distrupt the bleeding profile, whether blood and blood products (TDP, plateletes, colloid) were taken, whether the cardiotonic suppert were taken, as independent variables were recorded and all the results were compared between two groups.In 0.day and 3.day, received whole blood in with blue sitrate tube from the patients included in the study to work in TEG device. TEG data, R=reaction time, K=coagulation time, MA=maximum amplitude, MA60 and ?-angle, were recorded. As a result, all coagulation profile of patients, the whole mechanism from the beginning of fibrin formation to the period of strength of clot and resolution of clot, as well as all the haemostatic imbalance in patients was viewed with a full-blood as qualitative and quantitative.In this study, whether TEG device is an adequate method of diagnosis in coagulation disorders in patients with sepsis was evaluated. The coagulation profiles of sepsis and control groups were compared with five TEG parameters. Most of TEG parameters showed significient differences in both groups. We believe TEG will be usefull in evalution of coagulation disorders in patients with sepsis
