Abstract (EN)

Gumus MN. Potential drug interaction frequency and clinical reflections in the coronary intensive care unit of Van Yuzuncu Yil University, Dursun Odabas Medical Center, Van Yuzuncu Yil University Health Sciences Institute, Department of Pharmacology, Master of Science Thesis, Van, 2019. This retrospective study was carried out with the data, obtained from the hospital information processing unit, of 1523 patients hospitalized in the coronary intensive care unit of Dursun Odabaş Medical Center at Van Yüzüncü Yıl University between 01/06/2017-01/06/2018, after the necessary permissions and approvals were taken. The data of 1016 patients meeting the criteria of being hospitalized for at least 24 hours, ordered at least two medications on the same day and being in the age range of 18-80 years. 60382 drugs of 1016 patients were screened. From the drugbank.com and medscape.com database, a total of 4085 interacting drug pairs from 183 different interacting drug pairs were found. Five of the most interacting drug pairs were Acetylsalicic acid + Clopidogrel, Acetylsalicic acid + Metoprolol, Acetylsalicic acid + Bemiparin, Acetylsalicic acid + Enoxaparin, Pantoprozole + Clopidegrol. It appeared that the frequency of the number of interacting drugs did not depend on length of stay; but age and the number of drugs used. As the number of drugs increased, the frequency of interacting drugs increased, too. While the frequency of interacting drugs in the Netherlands was 40.4% in one study and 58% in another, this rate was 89% in a study conducted in Brazil. In our study, this rate was found to be 72.5%. Coagulation tests of 72 patients exposed to drug interaction were examined and aPTT values were found to increase. Only teamwork can help reduce high drug-drug interactions. This teamwork includes the drug interaction warning system which can be integrated into the hospital information system; a doctor who prescribes the benefit / harm assessment for the patient after paying attention to the interaction; a pharmacist who will regulate the drugs that the doctor has missed, and who knows the drugs that can interact during the application; and a patient-centered nurse avoiding to apply the drugs that will interact together. Otherwise, drug-drug interactions cannot be prevented no matter what precautions are taken.
