Abstract (EN)

Introduction and Purpose: Serious complications may occur in overdose use of Varfarin; one of the most commonly used oral anticoagulant agents nowadays. This study investigates demographic analysis of complications occurring in patients taking Varfarin and factors affecting mortality.Method: In the present study, the medical records of patients consulting the Department of Emergency, Dicle University Faculty of Medicine, in the period of September 2007- September 2010 as a result of overdose use of Varfarin, were analyzed retrospectively. For univariate analysis and catagorical variables chi-square test (?2), and for continuous variables Student t test were used. Average scores were calculated as Mean ± Standard Deviation (SD). P <0.05 was considered as significant. During the evaluation process, "SPSS 11.5" program was conducted.Results: It was found out that %57.6 of 151 patients consulting the hospital throughout the study period was treated in the emergency department. Among the patients followed as a high dose Varfarin, the most frequent additional disease with a rate of %42.3 was found to be IHD. The most common reason of use in patients was valve replacement with a rate of % 35 in number selected. With a rate of % 23.1 in total number, the most common complaint of all patients was subcutaneous hematoma. Uneducated ones comprised %60.9 of all patients. Patients in Group1 had been given erythrocyte suspension (ES), fresh frozen plasma (FFP), vitamin K. As for the patients in Group2 there were not any bleeding complications. Thus, after the doses of Varfarin were adjusted, they were discharged. As a complaint of application, intracranial hemorrhage was significant. All the patients with intracranial hemorrhage died. There was not a statistical difference between education level and mortality, however all patients who died were found to be uneducated. As a result of laboratory analysis of all patients, PTZ value was found to be significant with (p <0.001).Conclusion: Bleeding, due to a high dose of Varfarin in old ages, occurs with a high rate in those with comorbit illness, a lower level of education and those with a high value of PTZ.
