Abstract (EN)

Aim. In this study, it was aimed to evaluate the sociodemographic and clinical features, treatment and response time of 0-28 day-old patients who were followed up with the diagnosis of Persistent Pulmonary Hypertension in the Neonatal Intensive Care Service of Van Yüzüncü Yıl University, Department of Child Health and Diseases. Method. The study included 29 patients diagnosed with persistent pulmonary hypertension at 0-28 days, who were hospitalized between July 1, 2014 and July 1, 2019 at Van YYU Neonatal Intensive Care Service, followed by neonatal and pediatric cardiology. A p value of less than 0.05 was considered significant. Results. 58.6% (n = 17) of the patients are male, 62.1% (n = 18) were born by cesarean section. 13.8% (n = 4) 1 dose, 6.9% (n = 2) 2 doses of surfactant, 6.9% (n = 2) 2 doses of Celestone were applied to the newborns with PPHN. Mechanical ventilation was performed in 72.4% (n = 21) of the patients within the first 24 hours. Inotropically; 51.7% (n = 15) dobutamine, 34.5% (n = 10) dopamine, 6.9% (n = 2) digoxin, 6.9% (n = 2) milrinone, 3.4% (n = 1) adrenaline was applied, 34.5% (n = 10) no inotropic drugs were used. For the purpose of treatment, 72.4% (n = 21) sildenafil, 37.9% (n = 11) MgSO4, 31.0% (n = 9) NO, 6.9% (n = 2) ilioprost is given. As a result of the treatment, 65.6% (n = 19) of the people were treated successfully, 3.4% (n = 1) became chronic and 31.0% (n = 9) became ex. Treatment success does not change according to gender, mode of delivery, gestational age, blood pressure, age of diagnosis, APGAR 1st-5th min score, heart rate, concomitant diseases, infection and treatment administered (p> 0.05). Among the evaluated blood gas parameters, PO2 and HCO3 values were statistically significantly higher in patients with positive O2 saturation before and after treatment (p <0.05). It was found that the ALT value and CPAP application time were statistically significantly higher in the ex / chronicized group of patients (p = 0.03). The number of detected infections showed a low degree correlation with the age of diagnosis and the duration of MV statistically significantly (p <0.05). Conclusion. In PPHN treatment, O2 saturation, PO2, HCO3, ALT and CPAP application time parameters were determined to be important in predicting the treatment result. By determining these values in the early period, the prognosis of the disease can be estimated.
