Abstract (EN)

Neonatal sepsis is one of the most important causes of mortality and morbidity. Klebsiella strains are important factors for both early and late start neonatal sepsis. They cause serious morbidity, mortality, and high health care cost by leading epidemic in newborn intensive care unit. Broad spectrum cephalosporin and carbapenem usage is an important risk factor for infection progress. In this study, it was aimed to define the factors which effect morbidity, mortality, resistance patterns, Klebsiella sepsis frequency and related risk factor seen at the patients who stay in our newborn intensive care unit. Patients who were detected Klebsiella sepsis proven with culture in our newborn intensive care between January 2013 and March 2019 were included into our study. Patients' electronic files were examined retrospectively. Eighty four patients who were diagnosed Klebsiella sepsis proven with culture were included into our study. During the six-year study, Klebsiella sepsis rate was found 1,5% at hospital patients, and 19,8% at proven sepsis cases. Klebsiella pneumoniae (K. Pneumoniae) bred in 80 patients (95,2%) and Klebsiella oxytoca (K. oxytoca) bred in four patients (4,8%). Extended spectrum beta lactamase (ESBL) positivity was detected at 59,5% of patients. Low birth weight, prematurity, central catheterization, nourishment with TPN and long hospitalization were found as risk factors for klebsiella sepsis. The highest sensitivity in antibiograms were against retrospectively meropenem and amikacin (64,5% and 52,4%). The lowest sensitivity was belong to ampicillin (2,4%) and cefepime (11,9%). Mortality rates showed negative correlation with gestational week and birth weight (Respectively r=-0,751 and -0,663). In mortality developing cases, the highest sensitivity was detected against meropenem (75,9%) and amikacin (74,1%), and the highest resistance was detected against ampicillin (100%) and cefepime (96,4%). As a conclusion klebsiella infections were determined as an important problem in our study. Long hospitalization, low birth weight, prematurity, antibiotic usage before infection and invasive procedures become prominent as important risk factors. It progresses more mortal in very little premature and low birth weight neonatal. Obeying the barrier precautions especially medical staffs' hand hygiene, limiting invasive procedures, practicing infection control programs and rational usage of antibiotics are important to prevent infections.
