COMPARISON OF THE EFFICIENCY OF THE ALVARADO SCORING SYSTEM AND THE APPENDICITIS INFLAMMATORY RESPONSE SCORE IN THE DIAGNOSIS OF ACUTE APPENDICITIS Objective: Acute appendicitis (AA) is still one of the most common reasons for admission to the emergency department, but it is the most common cause of right lower quadrant pain requiring acute abdomen and surgery. In simple, new-onset AA cases, it can progress very quickly if it is not diagnosed quickly and effectively. And this progression may cause an increase in morbidity and mortality rates with perforation. Rapid and reliable scoring systems that help to diagnose prevent unnecessary surgeries, perforation rates and increase in hospital stay. In this study, we aimed to compare the effectiveness of the Alvarado Score, which has been proven to be reliable in many studies, and, in part, the newer AIR Score. Material- Method: This study was planned prospectively and 202 patients were included. 152 of them patients of these patients, AA surgery took diagnosis and 50 of the control group, to come to the emergency department with abdominal pain, surgical consultation was selected from patients who were not diagnosed AA but desirable. Working with our volunteers, age range 7-65, non-pregnant, it has been without a chronic disease, and patients who receive antibiotics within the last week. Symptoms, findings, physical examination and laboratory results were recorded. Data statistics were performed withIBM SPSS Statistics 19, SPSS inc ., An IBM Co. , Somers, NY. Results: Of the 152 patients in the AA group, 70 were 46,1% female and 82 were 53,9% male. The male / female ratio was found to be 1.17 / 1. The mean age was 29,18 ± 16 in the patient group, 39,42 ± 16,2 in the control group and 31,72 ± 16,61 in total. There was a significant difference between the two groups in terms of age variable p <0,001. The mean CRP value in the patient group 32,74 ±51,83 and 44,67 ± 51,63 in the control group, and no statistical difference was found between the groups p = 0,159. Alvarado score in patients of average 6,83 ± 1,75 in the control group and 3,06 ± 1,9 score 6,46 ± 2,51 The average patient groups were found to be 4 ± 1.73 in the control group and was statistically significant p <0,001. A one-unit increase in Alvarado Score disease enl of 5,836 to fold increase p <0,000, a unit increase in the score, an increase of over 0,634 fold disease p = 0,018, one-unit increase in CRP values has led to an increase on 0,986-fold disease p = 0,010. Our study evaluated when Alvarado Score 88% sensitivity and 80% specificity and 65% sensitivity and a score was found to have a specificity of 82%. The Alvarado Score is superior to the AIR Score for diagnosis. Conclusion: When the Alvarado Score and AIR Score activities were compared, the Alvarado Score was found to be a more meaningful, useful, simple, no extra cost and more successful diagnostic system .
