Abstract (EN)
Pulmonary thromboembolisms are one of the leading causes of mortality worldwide. Although the incidence of pulmonary thromboembolism cannot be calculated exactly because the presenting complaints are nonspecific symptoms, it is thought to be more than 60-70/100,000. It is thought to be responsible for 5-15% of all in-hospital deaths. For this reason, patients must be diagnosed quickly in the emergency department and treatment must be started. Clinical presentation to emergency departments ranges from a simple atypical chest pain to severe shock and respiratory failure. Although it is difficult to make a diagnosis because many diseases are included in the differential diagnoses, it is extremely important to make an early diagnosis as it can result in mortality. There is no biomarker specifically proven to be diagnostic in the literature. For this reason, we wanted to investigate the diagnostic value of the Oncostatin M biomarker, which is expected to increase in ischemic disease situations, in patients with pulmonary thromboembolism. Our study was monocentric and conducted prospectively in the emergency department of a tertiary university hospital. Our study included 44 PTE positive patients with a pulmonary thromboembolism clinic, 22 PTE negative patients and a control group of 22 people without a pulmonary thromboembolism clinic who met the inclusion criteria. In our study, the significance level was accepted as p <0.05. The average age of the patients included in our study was calculated as 64.76±16.08. Considering gender, 52 (59.1%) of the 88 individuals were male and 36 (40.9%) were female. There was no difference in age or gender of the patients between our groups. While the mean Oncostatin M was found to be 250.17±106.95 in our case group, it was found to be 130.79±71.33 in our control group. When the case group was examined within itself, the mean Oncostatin M was found to be 300.89±85.97 in PTE-positive patients, while it was found to be 148.723±63.99 in PTE-negative patients. These results were found to be statistically significant (p<0.001). While the diagnostic sensitivity of Oncostatin M levels was found to be 84.09% and the specificity was 90.91%, the predictive value was calculated as 225.5. According to the data obtained from our study, it is thought that the Oncostatin M biomarker can be used as a diagnostic test for pulmonary thromboembolism. Again, in our study, it was determined that Oncostatin M levels may be useful in indicating mortality. This is a preliminary study and we think that more detailed studies will provide clear information to the literature on this subject.
