Abstract (EN)
Purpose: The aim of our study was to analyze patients who were diagnosed and treatment for renal cell carcinoma. We analyzed the demographics, clinical, pathological characteristics, survival analysis and prognostic factors of cases and their treatment. We also aimed to view patients who received tyrosine kinase inhibitors Sunitinib and Pazopanib and determine the effectiveness, side effects and comparison of these patients. Method: Our study included the cases of 202 adult patients who received treatment for renal cell carcinoma between 2006 and 2020. Patients analyzed for demographic, clinical, laboratory, pathological characteristics and for treatment received as well as prognostic factors. We reviewed the histopathologic type of the tumor and the stage which the diagnosis was made. We studied the type of surgical treatment that was performed, as well as relapse and metastasis time and area. The response status of patients using tyrosine kinase inhibitors was evaluated. Comparison was made between the groups. We calculated the overall survival length and disease-free survival length and compared them between the groups. Results: Of the 202 participants there were 55 females (%27,2) and 147 males (%72,8). The median age was 57 years (21-83). The most common histologic tumor type was Clear cell carcinoma (%59). At time of presenting %57 of patients was early stage (Stage 1,2,3), %43 were stage 4 patients. Metastasis was present in %39 of patients. Early-stage patients had a median disease-free survival of 47.5 months, and a overall survival length of 82.5 months. For early-stage patients prognostic factors included stage of the tumor (p=0.002), MSKCC rick score (p=0.000), and ECOG performance score (p=0.002). Stage 4 patients had a median overall survival length of 16.8 months. Stage 4 patients had prognostic factors that included nephrectomy (p=0.001), Karnofsky score (p=0.000), MSKCC score (p=0,000), and ECOG score (p=0,000). In addition, it was observed that correct calcium elevation (p<0,001) and neutrophil height (p=0,012) shorten overall survival. Patients who received Sunitinib had a median progression-free survival length of 11.1 months, where as patients who received Pazopanib were 12.2 months. Both drugs did not differ in terms of survival (in order of p=0,278 and p=0,403), but less side effects and drug dose changes were observed in patients who received Pazopanib. Conclusion: Approximately half of the patients with RCC at diagnosis are detected at advanced stage. While the prognostic factors in the metastatic stage are especially the patient's performance score and the MSKCC score, in the early stage, the stage of the tumor at the diagnosis is added to these. There was no difference in efficacy between Sunitinib and Pazopanib drugs used in the treatment. It was concluded that Pazopanib is more advantageous in terms of side effects.
