Abstract (EN)

Aim: It is aimed to detect CD70, GNA13, BTG1, TNFRSF14, MYD88 and CD79b expressions frequently observed in diffuse large B cell lymphomas (DLBCL), and to determine whether these markers can predict prognosis in DBBHL cases by comparing these data with the prognostic data of patients. Material and Methods: A total of 52 nodal DLBCL, not otherwise specified (DLBCL, NOS) cases diagnosed between 1998 and 2017 with no known extranodal involvement at the time of diagnosis were evaluated. Clinical prognostic data were obtained in 44 of these cases. CD70, GNA13, BTG1, TNFRSF14, MYD88 and CD79b were studied by immunohistochemical methods and CD70, GNA13, BTG1, TNFRSF14 and MYD88 markers were excluded from the study due to inability of obtaining proper staining. The relationships between the results and clinical and laboratory prognostic markers were investigated. Results: It was observed that cases with CD79b expression >10 positive cells/LMA had lower overall survival (p = 0.002). It was observed that CD79b staining rates were higher in ABC subtype than GCB subtype. Also overall survival was found to be worse in patients with, > 60 years of age, ECOG performance score ≥2, stage IV disease, high IPI score (3-5), LDH elevation, low albumin level, low hemoglobin level, low peripheral blood lymphocyte count, high peripheral blood neutrophil / lymphocyte ratio, high peripheral blood platelet/lymphocyte ratio and MYC≥40%. Conclusion: Studies to determine new prognostic factors and develop targeted treatment methods in DLBCLs have increased considerably in recent years. In the literature, the evaluation of CD79b was predominantly evaluated by genetic analysis, and evaluation with immunohistochemical staining was observed at a lower rate. Since immunohistochemical staining is cheaper than genetic analysis, it is important to increase the studies in this direction. It was concluded that CD79b positivity in DBBHLs may be a useful parameter in predicting the prognosis of patients. The data suggest that Polatuzumab vedotin, an Anti-CD79b molecule currently in the Phase 1b/2 stage, can be promising in treatment.
