Among the leading causes of illness and death worldwide, diabetes mellitus (DN) ranks high. It has not yet been established what causes diabetic nephropathy. Only 30–40% of diabetic people will show signs of nephropathy. The objective of this study was to provide such estimates for DN and the presence of insulin antibody levels in the blood of patients DN, where samples were collected from 29 females, 44 men, and 30 healthy people (≥ 75 years of age). Our study was on insulin resistance and the formation of insulin antibody complex that is associated with kidney nephron destruction which is known as diabetic nephropathy. We detected no significant level of anti-insulin antibodies among these patients because our patients were not affected by autoimmune disease. Results: The study showed a positive correlation between Anti-Insulin Ab and serum creatinine, with microalbumin of DN patients (r: 0.284, P<0.01) (r: 0.398, P<0.01), respectively. The study showed a strong positive correlation of blood urea, serum creatinine, albumin/creatinine ratio, and HbA1c with anti-insulin antibodies in DN patients (r: 0.617, P<0.01), (r: 0.567, P<0.01), (r: 0.288, P<0.01), (r: 0.816, P<0.01) respectively. It can be concluded that the current study confirms the high prevalence of CKD in T2DM.there is no significant value in anti-insulin antibodies among diabetic nephropathy patients.
