The Frequency of Aspirin Resistance in Children with Heart Disease, Evaluation of Clinical and Laboratory Reflections Purpose: The aim of this study was; To determine the factors that may cause aspirin resistance in children with heart disease and to determine the frequency of aspirin resistance in pediatric patients with heart disease. Materials and Methods: Patients with cyanotic and acyanotic congenital heart disease who use aspirin for at least 8 days and had cyanotic and acyanotic congenital heart disease were included in the study, between April 2019 and October 2019 were followed up at the Pediatric Cardiology Policlinic of the Cukurova University, Faculty of Medicine. Results: A total of 38 patients (36.9%) had aspirin resistance. Aspirin resistance was detected in 22 (41.5%) of 53 cyanotic patients and 16 (32%) of 50 acyanotic patients. No relationship was found between sex, family history of thrombus, captopril, furosemide and stomach protection, enteric coated aspirin use, blood pressure and aspirin resistance. ROC analysis was performed to obtain a cut off value for fibrinogen. As a result of ROC analysis; The area under the ROC curve was found to be AUC = 0.81 (95% CI 0.72-0.89; p = 0.0001), sensitivity 86.8%, specificity 72.3%, and cut-off value 287.5. The mean values of albumin were found to be significantly lower in aspirin-resistant group than non-aspirin-resistant group. A cut-off value for albumin was desired, but no statistically significant result was obtained from the ROC analysis (p = 0.058). Conclusion: As far as known, studies on aspirin resistance in the pediatric population are limited. Some populations of children with heart disease are at risk for thromboembolic complications. The scope of this important problem in pediatrics is unclear. In addition, there is no data linking aspirin resistance to clinical outcomes in children. Therefore, it is vital to predict a possible aspirin resistance in children under antiaggregant dose of aspirin and to treat when detected.
