Karbamazepin ve valproik asit ile tedavi edilen epilepsili çocuklarda serum lipidleri ve karaciğer fonksiyon testlerinin incelenmesi

Objective: Aim this study; to determine the effects of carbamazepine (causes liver microsomal enzyme induction) and valproic acid (does not causes liver microsomal enzyme induction ) on the serum lipids and liver function tests in children with epilepsy, prospectively.Materials and Methods: İn this study, 128 patients whose followed at Dicle University Pediatric Health and Disease, Neurology department divided groups according to kind and period of antiepileptic drugs. This study; after to interrogate patients for some demographic specialities and potential side effects of drugs, is compared serum lipids and liver functions of patients, prospectively.Results: %55,47 of patients was treated with VPA and %44,53 of patients was treated with CBZ. The patients whose take this antiepileptic drugs divided according to periods drug for a short time (3 month>) and long time(1 year<) and compared serul lipids and liver functions.%57,8 of cases were male and %42,2 of cases were female. Age of cases changed 18 month to 15 years. Mean of age in VPA group was 7,25 ± 3,13 and in CBZ group was 6,54 ± 3,77 years.%10,9 of the first EEG were normal. When patients was interrogated according to potantial side effects of drugs;%4,2 of VPA group had nausea, %2,8 vomiting, %9,86 anorexia, %2,8 abdominal pain, %8,5 put on or lack weight, %1,4 ataxia, %1,4 vertigo, %2,8 nystagmus, %2,8 cutaneous refuses, %7,04 agitation, %4,2 tremor and %2,8 epistaxis.%7 of CBZ group had nausea, %7 vomiting, %7 abdominal pain, % 7 cutaneous refuses, %5,3 mild liver functions failure, %7 anorexia, %5,3 diarrhea, %1,7 diplopia and %3,5 ataxia. When serum lipid levels of the patients treated with VPA groups was compared, VLDL differed significantly. There was no significant difference serum HDL-C, TC/ HDL-C, HDL ?C, LDL-C,TG, T. cholesterol ve LDL-C/HDL-C levels. The patients treated with VPA had significantly different levels of GGT. Non significant difference was determined with respect to AST, ALT, albumin, T.bil ve ALP. When serum lipid levels of the patients treated with CBZ groups was compared, VLDL differed significantly. There was no significant difference serum HDL-C, TC/ HDL-C, HDL ?C, LDL-C,TG, T. cholesterol ve LDL-C/HDL-C levels. The patients treated with CBZ had significantly different levels of GGT. Non significant difference was determined with respect to AST, ALT, albumin, T.bil ve ALP.Conclusion:This study shows that blood level of VLDL and GGT were changed during VPA and CBZ treatment.But there are many studies to show increased serum lipid profile which coud lead to atherosclerosis during antiepileptic treatment.Long term prospective studies are required to evaluate the risk of atherosclerosis caused by alteration in serum lipid levels in children receiving therapy with antiepileptic drugs.
