Investigation of the factors affecting the development of pain, edema, and trismus during the extracti̇on of impacted mandibular third molar

INVESTIGATION OF THE FACTORS AFFECTING THE DEVELOPMENT OF PAIN, EDEMA, AND TRISMUS DURİNG THE EXTRACTİON OF IMPACTED MANDİBULAR THIRD MOLAR Objective: The surgical procedure of the impacted third molar is routinely performed in dental surgery. Inflammatory complications such as pain, swelling, and trismus have an adverse effect on the quality of life of patients after third molar removal. Bone removal is an essential step in a variety of oral surgical procedures. There is no literature regarding the optimum temperature of the irrigation solution during bone removal with steel round burs in oral surgery. This study was performed to evaluate how saline irrigation at different temperatures affects pain, edema, and trismus after impacted third molar surgery. Materials and Methods: Eighteen patients with bilateral symmetrical mandibular impacted third molars were enrolled in this split-mouth, randomized, observer-blind clinical trial. The patients were aged between 18 and 30 years. We used an irrigation solution at 4°C on one side and an irrigation solution at room temperature (25°C) on the other side. The amount of pain, trismus, and facial swelling were measured on days 2nd, 4th, and 7th. Results: The amount of pain, and facial swelling were not significantly different between the groups but had a lower value for cold irrigation than for room temperature irrigation at all postoperative periods (p>0.05). Regarding trismus, the value of max mouth opening for cold irrigation was higher than for room temperature irrigation at all postoperative periods. The trismus value was statistically significantly higher on the cold irrigation side (p<0.05). Conclusion: Cold irrigation therapy (4°C) has beneficial effects on the trismus after impacted mandibular third molar surgery.
