Abstract (EN)
This study aims to compare the effectiveness of manual and mechanical CPR using carotid doppler USG, ETCO2 and lactate. In this study of 54 patients, the mean age was 69.41±9.85 years. After CPR, 75.9% of patients achieved return of spontaneous circulation (ROSC). Almost half (46.3%) of the patients who achieved ROSC within the first 24 hours were exited, while the others (53.7%) remained stable. During CPR, the duration of USG measurements was longer in the first manual cycle and significantly shorter in the fifth cycle with the mechanical device. The change in PSV and EDV, ETCO2 and lactate values was more effective during mechanical CPR than manual CPR. In addition, lactate levels and EtCO2 values of exitus patients were significantly different from those who achieved ROSC. There were significant and positive correlations between PSV, EDV and ETCO2 changes of the patients, while there were significant and negative correlations between lactate changes and PSV, EDV and ETCO2 changes. PSV and EDV values had no significant diagnostic value in predicting the outcome of CPR, whereas ETCO2 and lactate were found to be significant. These findings suggest that mechanical CPR methods have better efficacy, carotid doppler USG and lactate can be used as a potential tool like ETCO2 in the evaluation of CPR, and the use of lactate together with ETCO2 may provide a clearer assessment in predicting the outcome of CPR.
