Abstract (EN)
Aim: Renovascular hypertension is an important etiologic factor among the causes of secondary hypertension. We aimed to research the frequency of the renovascular hypertension in essential hypertensive patients with microalbuminuria.Materials and Method:In 2009, totally 108 inpatients and outpatients who has essential hypertension with microalbuminuria and who had routine follow-up and therapy in Nephrology Clinic and also in department of internal medicine in the Education and Training Hospital of Medicine Faculty of Dicle University were enrolled into the study. Blood samples for biochemical, hormonal and hematological parameters were taken from patients. To determine Nephropathy, protein, creatinine and microalbuminuria levels were studied in urine of 24-hour. Glomerular filtration rates of patients were calculated using MDRD (Modification of Diet in Renal Disease) formula. ECG and ECHO of all patients were studied. Renal Doppler ultrasonography in all patients and renal ultrasonography were performed. 3 patients had MR Angiography.Findings: 307 hypertensive patients were screened and 108 (35.1%) of them has microalbuminuria. Renal artery stenosis was detected in 3 out of 108 (2.8%) essential hypertensive patients with microalbuminuria, following a renal artery Doppler ultrasound and a confirmation was made by MR angiography. The average age of the patients detected with RAS in our study was 33.67+/- 18.14, and their diastolic blood pressure was 95 + / -8.6 mmHg. In the statistical analysis of the patients with RAS, systolic blood pressure (p = 0.028) and urea (p = 0.048) were found to be significant.Conclusion: We found renovascular hypertension in 2.8 % of essential hypertensive patients with microalbuminuria. This rate is the same as the results acquired without studying microalbuminuria. We think that microalbuminuria has predictive value but has not diagnostic value in the progression of hypertension. However; a large-scale clinical studies are needed to conduct on this issue.
