The effect of hot-cold application to the bladder on postoperative urinary retention in orthopedic surgery patients

This research was conducted in order to examine the effect of hot cold application to the bladder on postoperative urinary retention in orthopedic surgery patients. December 18 2023-May 08 2024 randomized controlled intervention method conducted in the Orthopedics and Traumatology clinics of Ankara Bilkent City Hospital, the research was completed with a total of 88 patients, 44 in the intervention group, 44 in the control group. As data collection tools, the "Descriptive Characteristics of the Patients Form", the "Postoperative Urinary Retention Monitoring Form for the Intervention Group" and the "Postoperative Urinary Retention Monitoring Form for the Control Group" were used. All patients are postoperative 0-6.postoperative urinary retention symptoms (the presence and score of pain, desire to urinate, suprapubic pressure, increase in bladder volume, bladder globule) were evaluated between. Unlike the control group, patients in the intervention group underwent hot-cold application to the bladder, hot and cold once every hour, unlike the control group. Chi-square, Mann Whitney test and Friedman test were used for repeated measurements in the analysis of the data. p<0.05 was accepted for statistical significance. In the research, the median time of spontaneous urination of patients in the intervention group was calculated as 2 hours, and the median time of patients in the control group was calculated as 4 hours. All patients in the intervention group spontaneously urinated at the end of the 3.hour, but 32.5% of patients in the control group did not urinate spontaneously 4.the hour was determined that he still had not urinated. In addition, 4 patients (9.1%) in the control group experienced bladder globe, which was supported by other findings (desire to urinate, suprapubic pressure and increased bladder volume). In the research, it was concluded that hot-cold application to the bladder was statistically significantly different between the groups in terms of spontaneous urination, suprapubic pressure, bladder globe and postoperative pain parameters (p<0,05). As a result of the research, it was found that hot-cold application to the bladder was effective in preventing postoperative urinary retention in orthopedic surgery patients. For this reason, it is believed that nurses can evaluate patients in terms of postoperative urinary retention risk, prioritize noninvasive methods in the presence of bladder globus, and thus prevent complications that may occur as a result of invasive methods in preventing postoperative urinary retention, which is one of the complications of orthopedic surgery.
