The role of capnography in cases that urological endoscopic procedure was performed under sedation with propofol and remifentanyl

Abstract (EN)

Adult patients who undervent urological endoscopic procedures under sedation and analgesia were included in this study. In our study, we aimed to demonstrate that use of Capnostream technology decreases the complications of sedation and analgesia and increases the safety of patient. Patients were randomly divided into two groups that capnography was applied (Group I) or not applied (Group II). Patients were monitorized routinely. In Group I additionally, end tidal carbondioxide (ETCO2) and integrated pulmonary index (IPI) monitorizations were made. In both groups as starting dose 1 mg/kg propofol for sedation was followed 20-30 mg recurrent bolus propofol doses as needed. For analgesia 0.05 μg/kg/min remifentanyl intravenous infusion was administered and 0,5 mg/kg ketamin intravenous was applied as rescue drug. In Group II; SpO2, cardiac rate, respiratory rate, blood presure values; in Group I in addition to mentioned parameteres ETCO2 and IPI values were recorded just before the endoscopic procedure, at 3th, 6th and 9th minutes of the procedure. Nasal oxygen at 4 ml/min rate was given during the procedure to all patients. Complications occuring during procedure and airway interventions were all recorded. After the completion of endoscopy, patients were send to service when Modified Aldrete Score was ≥9. In our study significantly higher levels of cardiac rate at 3th minute were found in Group I. In Group I significantly higher levels of systolic, diastolic and mean blood pressures were found. Although there wasn't clinically significant as statistically among patients in Group II, we found that intraoperatively 6th and 9th minutes SpO2 values as statistically significant higher than Group I. Head tilt or jaw trust intervention number was significantly higher in Group I. While there wasn't shoulder roll practice in Group I, shoulder roll practice was applied in 3 patients in Group II. Sedation time was found higher in Group I. In conclusion, significant superiority of Capnostream technology using according to routine monitorization wasn't found at hemodynamic and respiratory parameters in our study. Further research is needed to better understand the clinical significance of Capnostream technology using.
