Main reasons that increase the speed of staff turnover in health institutions: A research on nurses

Nurses constitute an important part of health workers. Aging population, increasing life expectancy, increasing number of communicable and non-communicable diseases, increasing number of hospitals and health institutions increase the need for nurses in direct proportion. However, the World Health Organization predicts a shortage of 15 million health workers by 2030 and it is seen that nurses leave their jobs for some reasons. In this study, it was aimed to determine the reasons affecting nurses' turnover intentions and to examine the effects of job satisfaction, workload, work stress and work environment on nurses' turnover intentions. The study was conducted with 310 nurses working in four health group private hospitals on the Anatolian side of Istanbul, which have a large and intensive workforce, who gave their consent to the study on a voluntary basis. Data were collected using Personal Information Form, Minnesota Job Satisfaction Scale, Revised Individual Workload Perception Scale and General Work Stress Scale. The findings obtained in the study were analyzed with SPSS 25.0 program. Percentage distribution, mean, standard deviation, Cronbach's Alpha, Pearson Correlation and Regression analysis were used to evaluate the data. In the study, it was found that job satisfaction, workload, work stress levels were at medium level, staff turnover rates were at medium level and work stress levels were low. It was found that workload and stress had a statistically significant and positive effect on the turnover rate, while job satisfaction and working conditions had a statistically significant and negative effect. In order to reduce the turnover rate of nurses, it is thought that the number of staff should be increased, the number of patients per person for patient care should be reduced, and their work outside the patient care process should be reduced. It is predicted that improving working conditions and opportunities, increasing wages, following a fair wage policy and increasing motivational activities will be an important issue on nurses' turnover intentions.
