Abstract (EN)
Objective: In this study, we aimed to investigate the ability and effectiveness of mobilization at the postoperative 4 and 6 hours in patients who underwent spinal anesthesia for cesarean section and did not experience any perioperative complications. Materials and methods: After ethic approval, the study was conducted as a prospective randomized controlled study on patients hospitalized in Gynecology and Obstetrics clinic, Dicle University Medical Faculty Hospital. Randomization was performed on a weekly basis by dividing patients into two groups to be mobilized at the 4th hour and at the 6th hour. Patients between the ages of 18-42, who were pregnant for 24-42 weeks and underwent cesarean section with spinal anesthesia were included in the study. In the postoperative period, lower extremity muscle strength and sensory examinations were performed. Early oral hydration and early solid food intake were provided before the 4th or 6th hour. The relationship between lower extremity motor block initiation and end time, and the level of sensory examination and mobilization success of the patients who were mobilized at the 4th hour (n = 67) and 6th hour (Group 2) (n = 68) after spinal anesthesia was compared. Spontaneous micturition, gas and stool discharge, and time to hospital discharge were also compared. SPSS (Statistical Package for the Social Sciences) package program version 22 was used for the analysis of data. Results: Demographic data, measurements, laboratory parameters, obstetric parameters, cesarean indications, and vital signs were similar between groups (p> 0.05). There was no significant difference in the starting and ending times of motor block after spinal anesthesia. The success of mobilization was 88.2% in group 1, and 90.7% in group 2 (p>0.05). There was no significant difference between the groups in terms of time to first micturition, post-mobilization verbal numerical scale (VNS) values, incidence of post dural puncture headache (PDPH) frequency and duration of mobilization in the following days, and time until hospital discharge (p>0.05). There was a significant difference between the groups in favor of the 4th hour in the time until gas and stool discharge (p=0.001). Conclusion: It is not clear when patients who underwent cesarean section using spinal anesthesia will be mobilized for the first time in the literature. In our study, it was concluded that mobilization at postoperative 4 and 6 hours was safe and effective in hemodynamically stable patients.
