Research on the effect of preoperative period of respiratory rehabilitation on postoperative morbidity in thoracotomy and pulmonary resection

Abstract (EN)
Objective: To investigate the effects of short-time intensive pulmonary rehabilitation, pulmonary functions, complication rates and duration of hospital stay in patients with pulmonary resection with thoracotomy before and after surgery Methods: Sixty patients who underwent pulmonary resection with thoracotomy were included in the study and two groups were randomly assigned. Patients in the study group underwent intensive pulmonary rehabilitation for 3 hours per day for 7 days in the preoperative period. Standart maintenance was applied to the control group. Postoperative pulmonary rehabilitation was performed in both groups. Groups; spirometric pulmonary functions, respiratory parameters, blood gas parameters, complication rates, and hospitalization times. Findings: The demographic findings of both groups were similar when compared. The preoperative pulmonary function and blood gas parameters were similar (p> 0.05). No statistically significant difference was found between the parameters of respiratory parameters (FEV1, FVC) and blood gas (pO2, pCO2) in both groups after surgery. When both groups were compared in terms of complication rates, the percentage of non-complication development in the study group was higher (p = 0.028). According to other complications, prolonged air leakage development rate, which is common after thoracotomy and requires longer stay in the hospital, was found to be significantly lower in the study group (p = 0.005). Compared with the length of stay in the hospital, the study group was significantly shorter (p = 0.001). Comparisons of patients with smoking cigarette packs in the study group revealed that the duration of hospital stay in non-smoking patients was lower than in patients with 20 cigarettes / year (p = 0.019). Conclusion: When the data obtained from the study were evaluated, it was concluded that pulmonary rehabilitation applied in preoperative period of thoracotomy and pulmonary resection improved respiratory functions, decreased postoperative complication rate and shortened hospital stay.
