Abstract (EN)
Objective: Understanding the function of biomarkers of anomaly screening tests at the first and second trimester of pregnancy gave rise to several studies evaluating their convenience in prediction of diseases complicating pregnancy. Affecting metabolic events and growth, these parameters may also be related to metabolic disorders such as gestational diabetes. This study aims to evaluate the effect of these early screened parameters on 50-gram oral glucose challenge test at 24-28 week and on neonatal birth weight in order to predict patients and babies at risk of gestational diabetes. Material and Method: Records of pregnant women regularly followed in Obstetrics and Gynecology Clinic of Bilim University Europe Florence Nightingale Research and Education Hospital between January 2009 and December 2011 were analyzed retrospectively. PAPP-A, f-ßHCG, AFP, HCG, uE3, IA, justified MoM values of these anomaly screening test parameters, 24-28 week 50-g OGTT, and neonatal birth weight of 688 patients with singleton pregnancies satisfying the inclusion criteria were recorded. SPSS and Pearson correlation were used for statistical analysis. Conclusions: It was observed that the more the PAPP-A MoM value, the higher the neonatal birth weight (p<0,00001 r:0,158). HCG MoM is significantly related to neonatal birth weight (p=0,019 r:0,089), fasting blood glucose- FBG (p=0,019 r: 0,09) and OGTT (p=0.17 r:0,091). There is a statistically significant relationship between uE3 MOM and OGTT (p<0.001 r:-0,146). As gravida and parite increase, uE3 MoM value decreases (p=0,004 r:-0,109). As IA MoM increases FBG (p=0.001 R:0,129) and OGTT (p=0,041 r:0,078) also increase. Neonatal birth weight is a significantly related to both PAPP-A-MoM and HCG MoM, likewise OGTT is related to Ue3 MoM, IA MoM and HCG. Although there are statistically significant relationships among these parameters, level of correlation coefficient is low. Result: We can conclude that PAPP-A MoM values can predict the neonatal birth weight, women with higher PAPP-A MoM values can have over the average birth weight babies compared to the ones with lower values. It can be claimed that low uE3 MoM and/or high IA MoM values may foresee high FBG and OGTT values. in clinical practice, however, they are not appropriate to be used as screening tests due to low diagnostic power.
