The effect of transitional care model on complications causing readmission in patients undergoing open heart surgery

In this study, nursing care carried out in cooperation with a multidisciplinary team in line with the Transitional Care Model in patients undergoing Open Heart Surgery ; The study was conducted in a descriptive and cross-sectional type , which is included in the quantitative research design, in order to evaluate the effect of post-discharge follow-up of patients on the development of complications leading to hospital readmission . The population of the research consisted of 41 patients who had undergone open heart surgery in the Cardiovascular Surgery clinic of Private Medikalpark Ordu Hospital during the period of the research. Research data were analyzed using SPSS-22 for Windows package program. The descriptive information of the patient and the distribution of the patients according to their home follow-up status after discharge were expressed as numbers (n), percentages (%). Distributions according to reapplication status after discharge a . Fisher's Exact test and b. Analyzed using Chi-Square tests. As a result, in our study, it was seen that adult -elderly patients over 60 years of age who underwent open heart surgery encountered at least one health problem during their follow-up, and that the health care provided with multidisciplinary cooperation in line with the transitional care model provided ease in reducing the symptoms of the patients and coping with the problems they experienced. The problems experienced by the patients included in the care model were taken care of in line with the model and the problems were solved with the applications led by the transitional care nurse. Considering the complications leading to readmission in patients who underwent open heart surgery and were followed up with the transitional care model, hospitalization due to complications occurred in 7.32% of the patients and in 17.1% of the patients who underwent a standard discharge plan.
