Therapeutic and side effects of the antipsychotics agents: A naturalistic follow-up study

Aim: It was aimed to observe the drug-related effects and side effects of AP used inpatients in the child and adolescent psychiatry service at the admission time, 3rd and 6th months of hospitalization. Method: 94 participants hospitalized in the Child and Adolescent Psychiatry Clinic between 1 December 2020 and 30 May 2021 were included in the study. Diagnoses were made with K-SADS PL. For diagnosis; CGI-S, CDRS, SCARED, ABC, YMRS, SANS-SAPS, CY-BOKÖ; in order to determine the side effects of EPS, UHU, LUNSERS, ESRS, BAI, and AIMS were applied. In all controls; height, weight, and waist circumference were measured, hemogram and serum samples were taken. Results: Considering the diagnoses, 30.8% (n=29) MDD, 14.9% (n=14) ASD, 13.8% (n=13) BD are in the first 3 ranks. For APs; It was observed that 28.7% (n=27) were risperidone, 27.7% (n=26) olanzapine, 21.3% (n=20) aripiprazole, 8.5% (n=8) UE risperidone, 8.5% (n=8) clozapine and 5.3% (n=5) quetiapine. When the CDRS score of those with MDD was examined, those who used aripiprazole had a more significant decrease in the score compared to risperidone. When the YMRS score of those with BD was examined, it was seen that olanzapine decreased more significantly than UE risperidone. When the ABC score of those with ASD was examined, olanzapine decreased more significantly than aripiprazole. When the SANS-SAPS scale was examined in those with SCH and psychotic disorders, it was seen that clozapine had a more significant decrease in the score compared to olanzapine. There was a significant decrease in the CGI-S scores of the patients at 6 months. It was olanzapine that significantly increased glucose and HOMA-IR. Clozapine and olanzapine significantly increased total cholesterol and TG. UE risperidone significantly increased prolactin levels. When EPS side effects are examined with total scale scores, it is risperidone that causes significant side effects. ADR developed in 17% (n=16) of the participants, and ocular crisis was observed in 50% (n=8). ADR developed approximately 6.81 ± 5.71 days after hospitalizations. Conclusion: The effects and side effects of the APs used were evaluated longitudinally with various scales. It has been observed that APs are effective and significantly reduce the severity of diseases. However, it has been observed that there is a risk for many side effects due to APs and regular follow-up of children and adolescents using APs is required.
