Effect of mother's positions to women' S health on the first and second stage of labour: A meta-analysis study

Abstract (EN)

Objective: This study is conducted to determine the effect of upright and recumbent positions in the first and second stage of labour among women with and without epidural analgesia on women's health. Method: In this study, a meta-analysis which is systematic synthesis method was used. It is scanned accordance with predetermined keywords and inclusion criteria that articles, master's and doctoral theses between 1970-2015 years published on 11 databases. Risk of bias on studies was examined with Cochrane, publication yanlılık analysis was made if number of study is adequacy, the overall effect size was calculated with the risk ratio of effect size and the random effects model, heterogeneity tests and sensitivity analyzes were also performed. Results: After the scan has been reached 29094 resource, 45 studies were included to analysis according to inclusion criteria. In the first stage of labour without and wtih epidural analgesia, there were no statitistically signifigant difference in the spontaneous vaginal birth, instrumentel birth and use of augmentation. Women who were upright in the first stage of labour without epidural analgesia were less likely to have instrumental birth (RR= 0.625, % 95 CI= 0.416-0.940). There were no statitistically signifigant in the mode of birth of women with epidural on the second stage of labour. In the second stage of labour without epidural analgesia, while women who were upright less likely to have instrumental birth (RR= 0.683, % 95 CI= 0.506- 0.923) and episiotomy (RR= 0.811, % 95 CI= 0.723-0.910), postpartum blood loss rate was likely increased (RR= 1.386, % 95 CI= 1.081-1.775). Conclusion: In conclusion; it has been reached that upright position in women first stage of labour without epidural analgesia reduced cesarean birth. It was determined that upright position in women second stage of labour without epidural analgesia reduced instrumental birth and episiotomy while postpartum blood loss was increased. Women 's risk factors should be taking into account in deciding on the appropriate position on birth, it should also abandon the use of routine movement restrictions.
