Abstract (EN)
Objectives: Cytokines resemble hormones, peptide or glycoprotein structure chemical conduction molecules that mediate hematopoietic cells in the development and regulation of the inflammatory and immune response(2). There is a two-way relationship between the central nervous system (CNS) and the immune system, and this event is generated through cytokines(3). Studies today show that inflammatory cytokines and receptors are found in various areas of the brain, and local sources of synthesis are glia and neurons(4). It suggests that the increase in interleukin release due to disruption in neuronal homeostasis with various stimuli in epileptic patients causes disorders in the neurotransmitter system, causing epileptic seizures(5). Experimental studies have shown that cytokine activity is observed especially in the area where the seizure started and the discharge spread(6). Recent evidence suggests that IL-6 plays an active role in seizure formation and exacerbation,IL-10 is neuroprotective against the development of hypoxia-induced epileptiform activity in the hippocampus in mice,IL-17 was elevated in both CSF and peripheral blood in epilepsy patients correlated with seizure severity and increased proline levels with prolidase activity, leading to glutamate excitotoxicity,PG-E2 is known for its critical roles in neuroinflammation, neuronal hyperexcitability and excitotoxicity, local vasodilation, infiltration of immune cells, and regulation of many proinflammatory mediators in inflamed brain regions. At the same time, due to the effects of epilepsy on emotional, behavioral, social and cognitive domains, the relationship with psychiatric disorders has been of great interest. Epidemiological studies related to psychiatric disorders in epileptic patients indicate that psychiatric comorbidity is seen in up to 20-40%(7). Since these studies were performed using different patient groups and diagnostic tools, it makes it difficult to compare the methodological heterogeneity findings(8). In many studies, the control group was not included. In the studies with control group, it was reported that the patients with epilepsy had increased psychiatric problems compared to the normal control group. However, in these studies, it was noteworthy that there were few studies in which there were fewer patients or patients with other medical conditions as a control group and a comparison was made in the literature with a healthy group(9-12). Many studies report that epilepsy patients face many psychosocial problems and often cause depression, anxiety, and low self-esteem(13-14).In this study, in the light of the above-mentioned information, the levels of prolidase, PGE-2, IL-6,8,10,17 in the blood levels of patients diagnosed with epilepsy were measured by measuring the blood psychiatric assessment scale (BPRS), Arizona Sexual Experiences Scale (ASEX), Short Form (SF-36), We aimed to evaluate the Functionality Scale of Shortness (FAST), Beck Suicide Probability Scale in comparison with the healthy control group and in the light of current literature. Material and Methods: 45 patients and 45 healthy volunteers who were followed-up at the Dicle University Faculty of Medicine Education and Research Hospital Neurology Clinic and diagnosed with definitive epilepsy according to ILAE 1989 and 2017 criteria were included in our study. Blood samples were taken from the epilepsy patients and the healthy control group. Prolidase, PG-E2, IL-6,8,10,17 levels were crosscheked between the patient groups and the healthy control group. With the parameter studied, the age, gender, and the treatments they received were compared in comparison with BPRS, ASEX, SF-36, FAST, Beck Suicidal Idea - scales. Results:Blood values and psychiatric scales were statistically compared between the epilepsy patient and control groups by the T-Test and Kruskal-Wallis test. IL-17 was high in the control group (p <0.05). IL-6,8,10 was found to be significantly higher in the patient group (p> 0.05). The patient group was found to be significantly affected at all psychiatric scales. Conclusion: Results from this study show that seizures can alter cytokine levels in epileptic patients. However, it is not sufficient to interpret the validity and actual value of these effects in epilepsy pathogenesis and treatment, to measure the modulatory effects on the immune system and to confirm the findings. At the same time, the time of drawing blood from patients after the postictal period is different, reveals the concept of temporal changes in cytokine levels and affects the results of the studies. Unlike other studies, patients with primary epilepsy and seizures were taken under control. In terms of psychiatric scales, all results were significant and prove that patients with epilepsy are affected psychosocially.
